Оценка состояния жизненно важных органов у пациентов с тяжелой аутоиммунной патологией перед началом превентивной терапии туберкулезной инфекции.Методы. Проведен анализ медицинской документации, результатов клинического и лабораторного обследования 38 больных тяжелыми формами аутоиммунных заболеваний на фоне длительной иммуносупрессивной терапии, направленных в противотуберкулезный диспансер для исключения туберкулезной инфекции. Оценивали частоту и тяжесть коморбидной патологии, результаты гематологических и биохимических показателей крови перед началом превентивной терапии туберкулеза.Результаты. Показания к назначению превентивной терапии определены у 37 (97,4%) пациентов: у 7 (18,4%) были выявлены остаточные посттуберкулезные изменения; у 30 (78,9%) − латентная туберкулезная инфекция. Выявлена высокая частота значимой сопутствующей патологии (в среднем 4,63 нозологий на пациента) с преобладанием заболеваний желудочно-кишечного тракта (54,1%), органа зрения (54,1%) и опорно-двигательного аппарата (75,7%), потребовавших индивидуализации схем превентивной терапии и подбора доз препаратов. После однократного трехмесячного курса превентивной терапии у 27% пациентов отсутствовало угасание пробы с антигеном туберкулезным рекомбинантным, у 32,4% больных сохранялся гиперергический результат.Заключение. Проведение превентивной терапии туберкулеза у больных с тяжелой аутоиммунной патологией требует персонализированного подхода с индивидуальным подбором схем и доз применяемых препаратов, тщательного клинико-лабораторного мониторинга с последующим длительным наблюдением и оценкой необходимости повторных превентивных курсов.</p></abstract><trans-abstract xml:lang="en"><p>Aim. To assess the state of vital organs in patients with severe autoimmune pathology before starting preventive therapy of tuberculosis infection.Methods. Medical records, clinical and laboratory examination results were analysed in 38 patients with severe autoimmune diseases receiving long-term immunosuppressive therapy, which are referred to the TB dispensary to exclude tuberculosis infection. The frequency and severity of comorbid pathology, haematological and biochemical blood parameters before the start of preventive therapy of tuberculosis were assessed.Results. indications for preventive therapy were determined in 37 (97.4%) patients: residual post-tuberculosis changes were detected in 7 (18.4%), latent tuberculosis infection in 30 (78.9%). A high frequency of significant concomitant pathology (on average 4.63 comorbidities per patient) with predominance of gastrointestinal (54.1%), visual (54.1%) and musculoskeletal (75.7%) diseases was revealed, which required individualisation of preventive therapy schemes and correction of drug doses. After a single 3-month course of preventive therapy, 27% of patients did not experience fading of the skin test with a recombinant tuberculosis allergen; 32.4% of patients still had hyperergic results.Conclusion.
