Изучение чувствительности клинических штаммов комплекса M. tuberculosis к деламаниду с помощью автоматизированной системы Bactec MGIT.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. Изучено 79 клинических штаммов M. tuberculosis, выделенных у 78 больных туберкулезом и получавших лечение в 2017–2023 гг.; 39 штаммов с сохраненной лекарственной чувствительностью и 40 – с различными вариантами лекарственной устойчивости к противотуберкулезным препаратам. Определяли минимальные ингибирующие концентрации (МИК) деламанида в жидкой питательной среде Миддлбрука 7H9 с помощью системы Bactec MGIT, результаты чувствительности оценивали с применением критической концентрации 0,06 мкг/мл, рекомендованной ВОЗ. Для выявления мутаций, связанных с фенотипической устойчивостью к деламаниду у штаммов M. tuberculosis, применяли полногеномное секвенирование.</p></sec><sec><title>Результаты</title><p>Результаты. Выявлена высокая активность деламанида в отношении клинических штаммов M. tuberculosis, не подвергавшихся воздействию этим препаратом: значения МИК варьировали от 0,004 до 0,03 мкг/мл для 97,4% (76/78) изученных штаммов, что соответствовало чувствительности к препарату. Первичная устойчивость к деламаниду обнаружена у двух штаммов M. tuberculosis (2,6%; 2/78), выделенных от впервые выявленных больных туберкулезом органов дыхания; приобретенная – у одного штамма (2,6%; 1/39), выделенного от больного с хронической формой заболевания в процессе лечения лекарственно-устойчивого туберкулеза. Обнаружены генетические детерминанты устойчивости у трех клинических штаммов M. tuberculosis (мутации N91T и W88* в гене ddn и мутация Q299* в гене fgd1) с МИК, превышающими критическую концентрацию.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Aim</title><p>Aim. To study the susceptibility of clinical strains of M. tuberculosis complex (MTB) to delamanid using the automated Bactec MGIT system.</p></sec><sec><title>Material and methods</title><p>Material and methods. We studied 79 M. tuberculosis isolates from 78 TB patients treated in 2017-2023. 39 MTB strains were susceptibile and 40 had different drug resistance profiles to anti-tubercular drugs. Minimum inhibitory concentrations (MIC) of delamanid in Middlebrook 7H9 liquid medium were determined using the Bactec MGIT system, the susceptibility results were assessed using the critical concentration of 0.06 μg/ml recommended by WHO. Whole genome sequencing was used to identify mutations associated with phenotypic resistance to delamanid in MTB strains.</p></sec><sec><title>Results</title><p>Results. Delamanid demonstrated high activity against drug-naive MTB strains. MIC values ranged from 0.004 to 0.03 μg/ml for 97.4% (76/78) of studied susceptible MTB strains. Two MTB strains isolated from newly diagnosed patients with pulmonary TB had primary resistance to delamanid (2.6%; 2/78), and acquired resistance was detected in one MTB strain (2.6%; 1/39) during delamanid-based therapy for drugresistant TB.
