Abstract

The study based on the biannual experience (2010-2011) of the Moscow multidisciplinary TB-hospital’s surgery department. The indications to abdominal surgical interventions include: ventral hernias (43,9%), abdominal malignancy (16,3%), abdominal TB suspicions (15,3%) and cholecystitis and cholelithiasis (12,2%) – the same as in patients without TB. The most common operations were hernia repair (43,9%) and laparoscopy-assisted procedures (41,8%), more rarely – surgery on the stomach and esophagus (12,2%), on the bowel (7,1%) and on the pancreas (1,0%). The complications in the postsurgical period (6,1%) and the postoperative lethality (3,1%) not exceed the overage population rate. The chemotherapy based on the combination of four or five drugs for two or more months until
the TB stabilization allows avoid the TB exacerbation or relapse in the postsurgical period. In TB patients with concomitant abdominal malignancies the operative treatment is feasible in the short time, after two-three weeks intensive chemotherapy.

Abstract

One hundred years ago in the Kurkino village, near the Moscow, the zemsky (land-) hospital came into operation. It was founded by the widow of the prominent Moscow physician G.A. Zakhar’yn and designed by the famous Russian artist I.E. Grabar. In these buildings operated, in succession, zemsky hospital, military hospital, tuberculosis sanatoria (for children with bone and joints involvement and for adult pulmonary patients), anew thoracic military hospital, and since 1942 – the hospital for tuberculosis adult patients. During the century-long history, in the hospital worked many well-known Russian doctors, including S.S. Yudin, G.L. Gar, V.G. Shtefko, K.V. Pomeltzov, L.K. Bogush, N.G. Stoiko, M.I. Perelman and other. After the reconstruction, initiated by the Soviet Union Council of the Ministers in 1968, the hospital became the multi-field facility for tuberculosis patients. The hospital is well experienced not only in tuberculosis treatment, but also in thoracic surgery, otorhinolaryngology, gastroenterology, endocrinology, psychiatry, heryatry. In the hospital
the special departments for HIV-patients and multi-drug resistant tuberculosis were established. According “The Program of the Moscow city health service modernization”, in 2012–2013 the infrastructure of the hospital was developed, the new equipment was supplied (to the amount of 48 mln rubles), some buildings were overhauled completely. By the way of the Moscow tuberculosis service modernization, in the hospital structure were included facilities of the Moscow city Health Department: “Mental hospital № 16” and “Tuberculosis sanatoria № 58”. In 2014 the hospital was honorable mentioned by the Major of the city of Moscow S.S.Sobyanin for the longstanding contribution in the health care development and the highly skilled medical aid for Moscow population.
